Register as a New Patient

If you live within our Practice boundary you are welcome to register with us. Please click on the image opposite to use our interactive map and postcode checker tool.

If you wish to register with our surgery, please complete and sign the registration forms and questionnaires below and bring them into the practice. 

It is important that both you and all members of your household register at the same time.

If you are unable to download/print the forms please collect them from the surgery.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Please Note: It is mandatory that we have two signatures from you.  We will not be able to process any registration, book appointments or deal with medication requests unless the Registration Form and the Practice Charter page of the Questionnaire are signed.

To manage a patient's records on their behalf you will first need to fill in a proxy consent form. If patients can't access this they can ask for a form at the surgery

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Named GP

All practices are required to provide their patients with a named GP who will have overall responsibility for the care and support that the surgery provides for them.

Upon registering with the practice you will be allocated an accountable GP.  New and existing patients can obtain the name of their accountable GP by contacting the practice. If a patient requests a different named GP, reasonable effort will be made to accommodate their preference.

Having a named GP does not prevent you from seeing any other GP in the practice. At times your named GP may not always be available, so you may need to see a different doctor for same day advice or urgent appointments.
