Proxy Access for Children

This protocol sets out the process for allowing proxy access for parents of children below 16.

This is based on guidance from the RCGP toolkit:

FOR PROXY ACCESS TO BE GRANTED, BOTH THE PATIENT AND PROXY NEED TO BE REGISTERED AT THE PRACTICE

 

Parent requests access for child <16

If Child is below 11:

Child is not considered to have capacity to consent - proxy access will be given to parents upon registration

If Child is aged 11-15:

Child can be considered to have capacity to consent (Gillick competence)

  • Parents to complete and return this form - JTH Proxy Access Form (click to open)
  • Child and parent required to sign form
  • Signature of child needs to be WITNESSED by practice staff
  • Parents to bring in relevant documentation (below) when form returned
    • Birth Certificate ( or other appropriate document)
    • To confirm parental responsibility is held by those applying
    • Identification documents for those requesting proxy (Passport/ Driving licence)
    • Identification for child (Passport if available / photo ID if possible otherwise birth certificate)

Young Adult > 16:

Considered to have capacity.

Young adult needs to complete adult proxy form to allow access (unless there are potential issues with capacity such as learning difficulties (in cases of concern please speak with a GP))
